Enhancing Patient Access into General Practice – The NWL Model Across North West London, General Practices & Primary Care Networks were asked to develop an access model that enhances patients’ access to GP surgeries. The aims of the specification that North West London developed are to make effective use of resources, increase use of digital tools & increase proactive care for those that need it.
As part of the specification, we submitted a delivery plan, which includes:
- Capture patient voice and engagement in service development through engagement events and survey.- Completed patient engagement survey 02.2026
- Improve continuity of care for elderly and vulnerable patients- On going proactive engagements of advanced care planning clinics within practice.
- Answer 90% of calls within 10 minutes and respond to all clinical e-submissions by the end of the next working day.- Achieved and ongoing monitoring of practice performance .
- Expand digital access by promotion NHS app through engagement events and engage with local authority to enhance digital literacy
- Offer Enhanced. Access appointments at local hub suitable for those who work unsociable shift patterns- Achieved and regular appointments offered to patients.
- Identify patients with social vulnerabilities and provide support services relating to housing, employment, food banks, substance misuse and counselling.
